Evidence of Insurability

Introduction

Certain benefits, such as Employee and Dependent Life plans, will have a Guarantee Issue coverage value based on your company’s eligibility. This value is the amount of coverage an employee can elect without having to provide Evidence of Insurability.

Employees can also choose to elect a greater amount of coverage than the Guarantee Issue. In this situation, the employee will be taken to the Evidence of Insurability page during their walkthrough after they have chosen their benefits.

Evidence of Insurability Page

This page shows each of the benefits for which the employee elected a greater amount than the Guarantee Issue. It also directs them to print the form they will need to complete to provide Evidence of Insurability.
