Consolidated Enrollment Form

Introduction

The Consolidated Enrollment Form (or CEF for short) is a complete list of an employee’s profile and benefits elections. This form will be shown at the end of an employee’s enrollment walkthrough and is also available at all times through their Employee Menu in THEbenefitsHUB.

 

How to Find the Consolidated Enrollment Form

After searching for the employee whose CEF you need to view, the Consolidated Enrollment Form link can be found under the Benefit Plan Information icon. This link has been marked in red on the image below for the purposes of this article.

 

What Information Is Shown on the Consolidated Enrollment Form?

The CEF is split into several sections, depending on how your company has decided to configure your CEF page. In our training company, BrightPassage, the CEF displays the following sections:

  • Personal Information
  • Dependents Information
  • Beneficiary Information
  • Election Information

The image below shows the CEF for our favorite training company employee, Benny HUBster. The layout of the form is customizable, depending on the company, so bear in mind that yours may look different from this example. You can click each of the hotspot icons on the image () to learn more about what is being shown on the CEF.

You can also use the button in the top right corner of the image to open it in fullscreen. Press the Escape key on your keyboard to exit fullscreen.

 

Customizing the Consolidated Enrollment Form

As previously mentioned, the CEF can be customized depending on what you and your company would like to have displayed. While the Election Information is required to be on the page, the other available sections are optional. You can choose to add any or all of the following information to your company’s CEF.

  • Personal Information
  • Dependents Information
  • Employee Payroll ID
  • Emergency Information
  • Beneficiary Information
  • Signature

If you would like to add or remove any of the options here to your company’s CEF, contact your Account Representative to make that change!
