Electronic Statement of Insurability (ESOI)

Introduction

If your company offers a OneAmerica, Unum, or 5Star benefit to your employees, they will be able to complete their Statement of Insurability directly through THEbenefitsHUB. This process removes your part in approving or denying coverage, as the system will do so automatically based on the employee’s responses on their Statement of Insurability.

Remember, your employees must be the ones to fill these forms out. Neither system users, brokers, or you, as an administrator, have access to any of the pages after clicking “Begin SOI.”

 

Evidence of Insurability Page

If an employee elects coverage in their OneAmerica benefit beyond the Guarantee Issue value, they will be taken to the Evidence of Insurability page. Rather than printing a form from this page, they will be able to complete the form right then and there by clicking the ‘Begin SOI’ button.

 

Statement of Insurability

Acknowledgments Page

The employee is first brought to the Statement of Insurability Acknowledgments page, where they must sign off on some acknowledgments.

As with the System and Company Acknowledgments pages, employees should click the checkboxes to the left of each statement, then click the ‘I Acknowledge’ button to proceed.

Questions Pages

Next, the employee will be asked a series of questions that will determine their insurability. They should answer these questions by selecting the ‘Yes’ or ‘No’ radio buttons for each question and each family member that is being insured.

At the end of each page of questions, the employee should click the ‘Sign and Continue’ button to move to the next page.

Depending on their answers, employees may also need to fill out some additional information, as shown in the image below.

Coverage Summary Page

Once the employee has responded to all of the questions, their additional coverage beyond the Guarantee Issue will either be automatically approved, denied, or pended for insurer review. You, as the administrator, will not need to make any approvals or denials through the Employee Administration icon.

Note that, although you cannot access the Statement of Insurability pages or the employee’s answers as an administrator, you will be able to view the completion of the SOI stamped in the employee’s history file.
